Movement is the common cause. A floor that flexes, a shower pan that was not properly supported, or a change of material without an expansion joint will all crack grout in the same places repeatedly, and new grout in a moving joint cracks again.
Water is the other. Grout is not waterproof — the waterproofing in a shower is behind the tile, not in the joints — so grout that is constantly wet and staining can be a sign the layer behind it is not doing its job.
Where the grout has simply worn or discoloured with age and nothing is moving, regrouting and sealing is a genuinely good-value job and makes an old bathroom look considerably younger.
